Athletic Registration Forms

If you plan to participate in any Pasco School District Middle School or High School athletic program, please download and complete the forms below. Return the completed paperwork to your school's athletic office. Individual packets must be filled out for each child.
 
For High School athletics: English Packet, Spanish Packet
For Middle School athletics: English Packet, Spanish Packet
Concussion form is needed for all sports, and sports managers and trainers.
