All high school students (ages 14-17) need a signed Parent/School Authorization Form to work while attending high school.  

Instructions for students:

• Please bring your Parent/School Authorization form to Ana Tuiaea-Ruud, CTE Community Engagement Manager for the School Authorization verification/signature.
  • Office location-  Student Services, Room 1178
• If Mrs. Tuiaea-Ruud is not in her office, please leave the form with the Front Office staff
• The form will be ready for pick-up at the Front Office (usually 24 hours or less)

Todos los estudiantes de la preparatoria (entre 14-17 años) necesitan un Formulario de Autorización de Padres/Escuela firmado para poder trabajar mientras asistan la escuela preparatoria. 

Instrucciones para los Estudiantes:  

• Por favor entregue el formulario de Autorización Padres/Escuela a Ana Tuiaea-Ruud, Coordinadora CTE de Participación Comunitaria para la Autorización Escolar, verificación/firma.  
• Ubicación de la Oficina - Servicios Estudiantiles, Oficina 1178
• Si la Sra. Tuiaea-Ruud no se encuentra en su oficina, por favor de entregárselo al personal escolar de la Oficina Principal.
• El formulario estará listo para recoger en la Oficina Principal (normalmente en 24 horas o menos)
