School Immunization Attendance Requirements

Students entering school (preschool to 12th grade), attending in person or online, must get the required doses listed in the below charts to be in compliance with immunization requirements. School-aged students (preschool to 12th grade) attending before and after-school programs must meet the immunization requirements for their grade in school. The requirements follow the National Advisory Committee on Immunization Practices (ACIP) Immunization Schedule.

Certificate of Immunization Status (CIS)

Before a student enrolls in person or online, please provide a medically verified immunization record, your student’s health care provider can print this for you, or you can print it from MyIR, which is a free Department of Health online tool that allows families to view and print their official immunization records themselves. Go to myir registration to begin the sign-up process.

School and Child Care Exemptions from Immunization Requirements

Washington State Law allows parents or guardians to exempt their child from the school or childcare immunization requirements. Exemptions may be claimed for personal/philosophical, religious, or medical reasons. Measles, mumps, and rubella may not be exempted for personal/philosophical reasons. To request an exemption a completed Certificate of Exemption must be submitted to the school or child care center. More information about exemptions is available on the Department of Health website.
