Kindergarten Bus Transportation Form

Kindergarten Families,

To ensure that our kindergartens are safely transported to and from school each day, the PSD policy requires kindergarten students to have a parent or authorized caregiver at the school bus door in order for the driver to release the student. 

Kindergarten families, please complete the below form for each of your kindergarten students.  If you have any questions regarding this form, please feel free to contact the transporation department at 509-547-2510.
